1. Acceptance of Terms
By accessing or using the Kennesaw Primary Care and Psychiatry website (“Site”), you agree to be bound by these Terms and Conditions and all applicable federal and state laws and regulations. If you do not agree, you should not use this Site.
2. No Medical Advice / No Physician-Patient Relationship

We reserve the right to accept or decline any individual for services at our sole discretion, consistent with applicable laws and ethical obligations. Submission of an intake form does not guarantee acceptance as a patient or scheduling of services.
Patients must provide accurate, complete, and current information. Failure to do so may result in denial or termination of services.
4. Telehealth Services
If you participate in telehealth services:
- You consent to the use of electronic communications and understand associated risks (e.g., technical failures, data breaches despite safeguards).
- You are responsible for ensuring you are in a private, safe location during sessions.
- Services are provided in accordance with applicable Georgia laws and regulations.
- We reserve the right to discontinue telehealth services if clinically inappropriate.
- Telehealth services may not be appropriate for all conditions or emergencies.
5. No Emergency or Crisis Services
Kennesaw Primary Care and Psychiatry does not provide emergency or crisis intervention services.
If you are experiencing a medical or psychiatric emergency, call 911 immediately or go to the nearest emergency department.

6. Payments, Fees, and Financial Responsibility
Out-of-Network Status:
We are a non-participating provider with insurance plans unless explicitly stated otherwise. Payment is due at the time of service.
Patient Responsibility:
You are responsible for all fees, regardless of insurance reimbursement. We may provide documentation (e.g., superbills), but reimbursement is not guaranteed.
Estimates:
Any fee estimates are provided in good faith and are subject to change based on actual services rendered.
Missed Appointments & Cancellations:
At least 24 hours’ notice is required for cancellations or rescheduling.
Late cancellations or no-shows may incur a fee.
Repeated missed appointments may result in discharge from the practice.
We reserve the right to modify fees at any time with appropriate notice.
7. Electronic Communications & SMS Consent
By providing your phone number or email, you consent to receive communications from us, including:
- Appointment reminders
- Administrative messages
- Limited clinical communications (when appropriate)
Risks: Electronic communications (including SMS and email) may not be fully secure.
Your Responsibilities:
- Provide accurate contact information
- Notify us of any changes
- Understand that message/data rates may apply
You may opt out of non-essential communications at any time, though this may impact service delivery.
8. Privacy
Your use of this Site is also governed by our Privacy Policy. We comply with applicable Georgia privacy laws regarding protected health information.
